A 62-year-old man has died and two critical patients are being airlifted to hospital after prolonged entrapment following a head-on collision between a bakkie and a light motor vehicle on the N12 outside Potchefstroom on Monday evening.

ER24 spokesperson, Ross Campbell, said three other people sustained "moderate to serious injuries in the incident".

"A 57-year-old man and a 60-year-old woman needed to be extracted by the fire department from their vehicles," said Campbell. They were airlifted to a private hospital in Lenasia.

"The three other patients were treated and transported by other services to Potchefstroom provincial hospital. Unfortunately, nothing could be done for the 62-year-old man and he was declared dead on the scene."

Police are investigating.
