Former Independent Police Investigative Directorate boss Robert McBride was expected to testify at the inquiry.

Pretorius says McBride's failure to appear before the commission was caused by a mistake on the part of the commission's legal team. 

This comes after they failed to issue notices to people implicated on the matter. 

Political analyst, Karima Brown says this is a symptom of a flaw in the administrative capacity of the commission.

The proceedings are expected to resume on Thursday when McBride is expected to testify.
